Professional Cover Sheets by Industry

Healthcare (HIPAA)
Add the date and time (creates an audit trail), a patient reference number (not the patient name — keep PHI off the cover sheet), and the full HIPAA confidentiality disclaimer:
"This fax contains confidential information intended only for the named recipient. If you received this in error, you are prohibited from disclosing, copying, or using its contents. Please destroy it immediately and notify the sender."

Never put PHI on the cover sheet

Legal
Include your bar number, the client matter/file number, and a privilege header:
"ATTORNEY-CLIENT PRIVILEGED COMMUNICATION — This communication is confidential and intended solely for the named recipient."
